Purchase, Deposit & Cancellation Policy

BOOKING AND DEPOSITS
A non-refundable deposit is required to reserve your appointment:
- Cardiovascular Screening ($297): $50 deposit at booking
- Cardiovascular Screening + Full Blood Panel ($697): $100 deposit at booking

Your appointment time is held exclusively for you, and we turn away other patients for that slot. For this reason, deposits are non-refundable under all circumstances. Your deposit is applied toward your total. By booking, you authorize ProActive Health Screening to charge the remaining balance ($247, or $597 with the blood panel) to the card used at booking after your appointment is completed. HSA and FSA cards are accepted.

RESCHEDULING
You may reschedule at no charge with at least 48 hours' notice, and your deposit will carry over to your new appointment. Call us at (657) 210-4547 or use the reschedule button in your confirmation email.

CANCELLATIONS AND NO-SHOWS
Deposits are non-refundable for cancellations, late cancellations, and no-shows. Cancellations or reschedule requests made less than 48 hours before your appointment forfeit the deposit. Arriving more than 15 minutes late may require rescheduling, and the deposit will not carry over.

BLOOD PANEL
If you added the full blood panel and your blood has already been drawn, the blood panel portion of your total is non-refundable, as lab processing costs have already been incurred. Your lab results will still be provided to you.

IF WE NEED TO CANCEL
If we cancel or reschedule your appointment for any reason, your deposit will be applied to a new appointment time of your choice, or refunded if you prefer.

BOOKING FOR SOMEONE ELSE
If you booked for a family member or friend, the appointment may be transferred to another person with at least 48 hours' notice.
